This form is an interactive PDF, meaning you can enter your details directly into the form by clicking on the highlighted boxes. You will still, however, need to print the form off to sign and complete your credit card details. This is required to ensure your credit card details remain secure.
Once you have completed the form, please ensure you have included a copy of your son’s birth certificate and, if both parents were born overseas, include proof of citizenship. Please provide a copy of your and your son’s visas, Australian passport or Australian citizenship certificate. Please email these to: admissions@hale.wa.edu.au.
